Once your infernal has aggro, he's not likely to lose it no matter how much you hammer on Karam as long as long as you don't command it to attack anything else. As soon as he's hostile, spam CTRL-1 (or whatever "attack pet" is keybound to) to get your infernal to aggro Karam. Target Karam and wait for him to become hostile you'll know he is when he knocks you back 20 yards and 2/3s off your health bar, but don't panic.
Position yourself such that you and your infernal stand directly in front of Karam, between him and Raest. Start by summoning your infernal, set it to follow, and "pacifist," meaning it won't attack anything until you command it. My secondary stats were split as follows during my success run: * Bring bloodlust drums, Prolonged Power potion, Whispered Pact flask, and Azshari Salad (haste food).
